Do you know about PANDA’s Supplemental Disability Registration Form?

PANDA’s Supplemental Disability Registration Form can help determine if a student has a disability. Simply add the question on the right to your current registration form.

If the student answers “yes”, complete the Supplemental Disability Registration. This form, which is available in English, Spanish and Somali, asks specific questions about an individual’s disability needs and accommodations they may need in the classroom. There is also a question about documentation of their disability, such as, “Do you have an Individualized Education Plan (IEP), 504 Plan, medical or mental health professionals’ letters?”

Gathering this information will help you better serve the student by determining if they are an appropriate fit for your ABE program or what accommodations they may need in the classroom.

The last page of the intake is a consent to release information. Ask the student to sign it if they have a case worker or social worker. The consent will allow you to contact the student’s support person to obtain more information about the student’s potential needs/accommodations in the classroom.
